Overview
The PhotoBiz Forms & Contracts tool allows you to create custom forms, questionnaires, agreements, and contracts for your clients. Adding a Signature Block gives your clients a way to electronically sign a form by typing their name.
This is commonly used for model releases, event contracts, session agreements, consent forms, and other documents that require client approval. This guide walks you through how to add a Signature Block to a form or contract and explains what your client will see when they complete it.
A Signature Block can be used when you need a client to confirm, approve, or agree to information submitted through a form.
Common uses include:
The Signature Block is a typed electronic signature field. Clients type their name into the field, and PhotoBiz displays the typed name in a signature-style format on the submitted form.
Log in to your PhotoBiz account and click FORMS from the left-side menu. This will open your Forms & Contracts dashboard, where your forms and contracts are stored.
At the top center of the Forms Dashboard, click FORMS to access the form builder. This is where you can view, create, and edit your forms and contracts.
Locate the form or contract where you want to add a signature field. You can scroll through your list of forms, browse your form categories, or use the search bar to find the form quickly.
Click the form name to open it in the editor.
If you are using a built-in template, such as a Model Release or Event Contract, review the form first. Many contract templates already include a Signature Block.
In the form editor, look at the right-side panel where the available form blocks are listed. Scroll to the MISCELLANEOUS section and click Signature Block.
The Signature Block will be added to your form. Your changes are saved automatically.
Your form is now ready to collect electronic signatures. To confirm everything is working correctly, preview the form or submit a test entry. After submission, the signature should appear in the saved form record.
When your client opens the form, they will see a signature field where they can type their name. After they submit the form, the typed name appears in a signature-style format on the saved submission.
This allows the completed form or contract to include a clear electronic signature from the client.
How the Signature Field Works
The Signature Block allows clients to type their name as an electronic signature. The typed name is then displayed in a signature-style format on the completed form or contract.
This field is ideal for forms that require acknowledgement, consent, or approval. It can be used on both regular forms and contract-style forms.
Before using a form as a contract, review the wording in the form to make sure it includes the agreement terms you want your client to accept. PhotoBiz provides the Signature Block, but you are responsible for the wording and terms included in your form or contract.
Important Notes About Submitted Forms
Once a form or contract is submitted, the submitted information cannot be edited or changed. This includes the signature field.
If a client needs to make changes after submitting a signed form, they will need to complete and submit a new form.
Submitted forms can be viewed at any time in your Form Submissions area. You can also use the Document View option to view or print a formatted version of the submission.
Using Templates
Built-in contract templates often already include a Signature Block. Reviewing these templates can save time when creating common forms such as model releases, event contracts, and client agreements.
You can also use sample forms to see how the Signature Block appears to clients. Clients type their name into the signature field, and PhotoBiz displays the typed name in a signature-style format after submission.

Why don’t I see the Signature Block on my form?
Make sure you are editing the form in the form builder and looking in the right-side block panel. The Signature Block is located under the MISCELLANEOUS section.
Can clients draw their signature?
No. The Signature Block is a typed electronic signature field. Clients type their name, and PhotoBiz displays it in a signature-style format.
Can a submitted signature be edited?
No. Once a form or contract is submitted, the submission cannot be edited. If the client needs to correct or update their signature, they must submit a new form.
Can I use the Signature Block on both forms and contracts?
